Creating a natural hairline is one
of the most important elements of
a successful hair replacement surgery
and is at the same time a real challenge
for the operating hair transplant surgeon. The
hairline defines the result in hair
replacement surgery and is almost like the
hair transplant surgeon's signature. The work
of the hair transplant surgeon is almost artistic;
the more skills and experience the
hair transplant surgeon has, the more natural
the appearance of the hairline will
be. Designing a new hairline requires
close deliberation between patient
and hair transplant surgeon. Patients often have
a tendency to desire a very low hairline.
However, they should keep in mind
that the pattern of the hairline has
to look appropriate twenty or thirty
years after the hair replacement surgery. A low, adolescent hairline
will look unnatural on a 55 year-old
man; he would sooner resemble a caveman.
This proves the importance of finding
the accurate hairline design for every
patient, irrespective of age. The
hair transplant surgeon should explain the benefit
and value of a more conservative pattern
to his patient: a hairline that will
both look natural at presence and
after many years. It is easier to
lower a high hairline than vice versa.
It is always possible to lower the
hairline in a second procedure by
adding follicular unit grafts.
To
arrive at an aesthetically natural hairline,
the first 0.5-1.5cm of the transitional
zone (at the interface of skin and hair)
will by transplanted only with follicular
unit grafts of a soft and irregular
pattern. The defined zone of the hairline
lies immediately behind the transitional
zone and is about 2 to 3cm of width.
The double hair grafts are used to approach
the natural density of the hairline
as closely as possible. The triple hair
grafts are usually inserted in the balding
zones of the area of the crown or to
"fill in" the defined zone.
